At ACE Supports, we are a dedicated NDIS-registered provider, delivering personalised support to individuals with intellectual disabilities, Autism, younger onset dementia (YOD), mobility impairments, mental health, and other disabilities. Our services are designed to provide flexibility and choice, including in-home support, community access, and group social activities.

Our Hospital Transitioning Service ensures participants move safely from hospital to home, Supported Independent Living (SIL), or Specialist Disability Accommodation (SDA) with structured supports that minimise hospital readmissions and ensure continuity of care. We also assist in developing tailored Care Support and Action Plans, allowing participants and their families to transition smoothly with the right support in place. 

ACE Supports goes beyond traditional care by offering memory support for individuals with dementia, household maintenance, domestic assistance, and social engagement programs. Our Short-Term Accommodation (STA) options provide participants with support in a safe, structured environment, while families benefit from essential respite, knowing their loved ones are receiving high-quality care.

Our services cater to all areas of disabilities, including complex care needs requiring a higher level of support due to disability, chronic conditions, acquired brain injury, and younger onset dementia, except for high-intensity supports that require specialised, health-related management. By concentrating on this diverse age group, we create a dynamic environment where participants can thrive, learn, and embark on a path to independence.
